This form is used to submit your Public Service Loan Forgiveness (PSLF) application to UD's Payroll to complete the employer Section (3).
Note: You can also send the form to be electronically signed by using the PSLF Help Tool located on the Federal Student Aid website listed above. Please ensure that the employer section is designated as University of Delaware, EIN 51-6000297 and that you enter the correct hours per week (average) that equates to the standard hours for your position. We will not be able to sign the form electronically if those fields are not correct.

Where can I get the PSLF form and what sections should I complete?

How do I submit my PSLF form?
Please attach the completed PSLF in PDF format.
Please ensure that the form is encrypted with your SSN as password before attaching it. We will complete your employment information as provided by your department, sign and return the form to you to submit to the US Department of Education.
