Accessing your Performance Appraisal Form

​If you are a full-time, benefited employee, you can use this link to access your form.  University of Delaware Central Authentication Service - Login.  NOTE: For new staff, the form may not be accessible and depends on the individual’s start date. 

​If you cannot locate an appraisal form, you can change the sort order of the appraisals using the “Appraisal activity”, “Status”, and “Current holder” headings to quickly find this year’s form. 

​Locked Appraisal Forms 

​The appraisal system will lock after a long time of inactivity, or if you click the “X” on your browser instead of clicking “log out”.  Also, avoid having more than one tab open when working on your appraisal.  If this occurs, wait 30 minutes, and then log back on. 

​Adding your New Supervisor   

​ Delete the previous supervisor, then search for and select your new supervisor. 

​Adding a Reviewer   

​Add reviewers by clicking the ‘Review & submit’ tab, scroll down, and enter the name of the reviewer under action ‘Add a reviewer’.   Be advised that reviewers can enter their own narrative; however, only the primary supervisor is able to select a rating. 

​Updating/Changing your Appraisal Form after Submitting to your Supervisor

​Contact your supervisor and request that your appraisal be returned to you for editing.  If the form is with HR and not yet completed, email hr-appraisals@udel.edu to ask for the appraisal to be sent back to the employee or the supervisor.   

​Draft Appraisal Forms for Separated Employees 

​The supervisor should email hr-apprasials@udel.edu to request the appraisal to be canceled. 

​Adding More than One Supervisor to Provide Feedback 

​More than one supervisor can be added to an appraisal; however, the first listed supervisor is primary. This means the primary supervisor will be able to add rating and narrative, and the subsequent supervisor can only enter a narrative.  Talk to your supervisor to see who will be primary as far as assessing your performance for the past year. 

​Edits Once the Form has been Approved by HR

​Once the appraisal form has been approved by HR in the system, it is no longer editable.  

Accessing Past Performance Reviews for Direct Reports

​If you did not author the appraisal (e.g., a previous supervisor did), please contact your HR Partner who will be able to assist you with this.
