1095-C forms for retired employees are provided by the State of Delaware. Direct your inquiry to the State of Delaware.
The Payroll Office only issues 1095-C forms to full-time benefited employees. Questions about Student Insurance plans and 1095-B forms should be directed to University Health Plans info@univhealthplans.com or call 800-437-6448.
Please be advised, 1095-C forms are NOT required to file your IRS Federal taxes. Refer to the Payroll Website and/or Affordable Care Act (ACA) for additional information. UD will be mailing 1095-C Forms no later than the IRS Deadline of 3/3/25.
