To change your bank account information for your retiree medical/dental/vision deductions, please complete a new ACH form https://www.udel.edu/content/dam/udelImages/human-resources/benefits/retirementforms/ACH_form.pdf, and upload it to the secure document portal https://www.udel.edu/faculty-staff/human-resources/forms/.
Human resources will reach out to confirm that it has been received. If you prefer, you may mail it to the address on the top of the form. Please include a copy (or scan) of a voided check. Please do not email this form directly for your security. We also cannot update account information over the phone.
